See 5-6 patients per day during an eight-hour day conducting Annual Medicare Wellness Exams.
Complete a physical examination, head-to-toe assessment, medication reconciliation and risk assessment
Seek out gaps in care, educate patients on nutrition and document any findings or testing recommendations
You will have the ability to create your own schedule between the hours of 8am and 8pm Monday through Sunday. You will have the ability to work an eight or ten-hour day. Optum will never ask you to work nights/weekends/holidays and there is no call. You will have up to an hour with each patient. Charting is completed during your visits on a tablet that Optum provides.
This role offers complete autonomy, and you will work on a team of Nurse Practitioners that is managed by Nurse Practitioners. Our platform offers a wellness-based care plan that offers our clients services in the comfort of their homes. Many of these patients do not see their PCP's, and you are the provider that bridges the gap.
