Job Description

Are you a compassionate and skilled Licensed Practical Nurse (LPN) looking to make a meaningful impact on women's health? We're seeking a dedicated LPN to join our dynamic team, where you'll play a vital role in providing exceptional care to our patients.

This role offers the opportunity to work alongside experienced doctors, nurse practitioners, and other healthcare professionals in a supportive and collaborative environment. We are open to both full-time and part-time candidates and offer a flexible schedule, with the potential for remote administrative days.

Your Role and Responsibilities:

As a key member of our team, you will:

Provide Essential Patient Care: Gather vital information to support provider assessments, administer prescribed medications and treatments, and monitor and record vital signs, medical history, and symptoms.
Assist in Medical Procedures: Support physical exams and medical procedures within your licensure.
Empower Patients Through Education: Provide comprehensive education to patients and their families on various healthcare topics.
Maintain Accurate Documentation: Ensure timely and accurate documentation of patient care.
Collaborate with External Partners: Work closely with school nurses, WIC colleagues, DCF, and other medical departments, completing necessary paperwork for external organizations.
Provide Expert Triage: Triage patient calls using established protocols to determine appropriate care, including at-home management, and scheduling medical visits.
Offer Comprehensive Patient Support: Educate patients and families on after-hours care, triage and referral services, and disease states.
Coordinate Specialty Care: Contact specialists to obtain letters and results, and assist in setting up home therapies as directed by providers.
What We're Looking For:

High School Diploma or GED.
Active LPN license in the state of Connecticut.
Previous experience in pediatrics is highly valued.
Exceptional communication and interpersonal skills; bilingual candidates are strongly encouraged to apply.
Ability to thrive in a fast-paced environment.
Strong attention to detail and multitasking abilities.
Solid understanding of medical terminology and nursing protocols.
Experience with electronic medical records (EMR) is required; experience with eClinicalWorks is a plus.
