Overview RN Case Manager Are you a compassionate and dedicated Registered Nurse looking to make a meaningful impact in the lives of others? As a Home Care RN, you will:
  • Deliver exceptional nursing care to clients in their homes, ensuring they receive the highest standard of medical attention and support.
  • Collaborate closely with a multidisciplinary team, including therapists, social workers, and other healthcare professionals, to provide comprehensive care tailored to each client's needs.
  • Follow physician's orders and work under the guidance and supervision of a Clinical Supervisor to ensure all medical protocols and care plans are meticulously followed.
  • Build meaningful relationships with clients and their families, offering not only medical care but also emotional support and reassurance during their health journeys.

  • Skilled Nursing Care and Comprehensive Assessment
  • Teaching, Supervision, and Counseling Clients and Caregivers
  • Interdisciplinary collaboration
  • Problem solving and critical thinking skills
  • Monitors patient progress to ensure accomplishment of patient goals
  • Oasis documentation
  • Provides appropriate care for a range of physician-ordered treatments (PleurX Drain, Finger stick Coagulation checks, wound care, wound vacs, IVs, phlebotomy, Trachs, Peg Tube, Ostomies, Telehealth)
Qualifications Minimum of one (1) years recent experience as a professional nurse in a clinical setting. Home Health experience preferred. Must complete Point of Care Testing training as part of Department Orientation. Competencies Skills Essential:
* Experience In Hospital Or Acute Care Setting
* Clear Communication Skills Both Written And Verbal
* Able To Keep Confidential Information Regarding Patients, Team Members
* Able To Withstand Crisis Situations
* Has Skills To Provides Customer Service To Patients, Team Members And Visitors
* Proof Of Current/Valid Car Insurance
* Knowledge And Experience With Electronic Health Records
* Valid DL With No More Than 3 Moving Violations

Credentials Essential:
* RN - Registered Nurse
