Job Description

Summary:
2-3 visits per day, within the same area. The position entails the completion of initial enrollment assessments and reassessments with focus on clinical, psychological, and social needs of the prospects as well as existing members utilizing Uniform Assessment System for (UAS-NY) in accordance with DOH approved policies based on New York State regulations. Assessment Nurse, as part of the interdisciplinary care team is also responsible for completion of DOH approved forms during the face to face visit, which will be incorporated into the Individualized Person Centered Service Plan.

DUTIES:

Perform home visits to conduct functional, psychosocial and environmental safety assessments, determine eligibility of enrollees, and establish an individual plan of care that enables them to remain safely at home despite long-term care needs. 

Assessment RNs will ensure that Maximus/DOH UAS information is complete and accurate as well as conducting follow-up assessments with the members to ensure the members are receiving the proper care and services.

MIN QUALIFICATIONS:
RN, UAS, MLTC CM, or New Grad, Car & Driver's License for some areas, language for some areas may be required.

• Current NYS RN license and registration
• UAS Certification is a plus
• Bachelors of Nursing degree preferred
• Experience in UAS, Community Health nursing, Managed Care, UAS Certified
• Working knowledge of Medicare, Medicaid and NYS DOH standards and regulations preferred
• Car and driver license for some areas
